SPECIAL HANDICAPPED PROGRAM AND NURSING HOMES PROGRAM (R.S. 18:1321 et. seq.) This program allows eligible voters who are homebound to vote absentee by mail and allows nursing home residents to vote in person at the nursing home or absentee by mail.  The Voting Rights for the Elderly and Individuals with Disabilities brochure is provided as a guide for eligible voters who wish to participate in the program, for the election officials implementing the program, and for any other individual who may have an interest in the law. To apply for the program, you may use the forms below.  You will need to submit to your parish Registrar of Voters a Request Form, an Authorization Form, a Physician's Certificate certifing that you are homebound, if applicable, a Witness Form, and an Acknowledgement Form.
